One Cycle Plus IVF Package
1 Retrieval and up to 2 embryo transfers (fresh or frozen)
Includes:
- Office Visits
- Ultrasounds
- Blood Work
- 1 Retrieval
- One fresh and one frozen embryo transfer (FET) or two FETs.
- Clinical care following the one IVF cycle until a serum pregnancy test is performed.
- If a pregnancy loss occurs during the nine-month benefit period, additional infertility services are provided through the balance of the nine-month period, up to the maximum of services.
Two Cycle Plus IVF Package
Up to 2 retrievals and up to 4 embryo transfers (fresh or frozen)
Includes:
- Office Visits
- Ultrasounds
- Blood Work
- 1 Retrieval
- One fresh and one frozen embryo transfer (FET) or two FETs.
- Clinical care following the one IVF cycle until a serum pregnancy test is performed.
- If a pregnancy loss occurs during the nine-month benefit period, additional infertility services are provided through the balance of the eighteen-month period, up to the maximum of services.
Three Cycle Plus IVF Package
Up to 3 retrievals and up to 6 embryo transfers (fresh or frozen)
Includes:
- Office Visits
- Ultrasounds
- Blood Work
- 1 Retrieval
- One fresh and one frozen embryo transfer (FET) or two FETs.
- Clinical care following the one IVF cycle until a serum pregnancy test is performed.
- If a pregnancy loss occurs during the nine-month benefit period, additional infertility services are provided through the balance of the thirty six-month period, up to the maximum of services.
